Patient's Profile

Ms NX, in her early 40's, is a non-smoker who leads a healthy and active lifestyle. 

Background 

In 2023, Ms NX consulted Dr Puah regarding a bothersome hump around her neck and shoulder region that seemed to increase in size as years went by. It was accompanied by a debilitating pain that was disrupting her daily life. Ms NX's self-confidence was affected, and she avoided clothes that exposed her neck and back. She tried various methods, including massage and many non-surgical fat reduction treatments, but to no avail. 

Consultation & Assessment

A detailed medical history and examination was undertaken. Ms NX had no underlying medical condition such as Cushing's syndrome, nor was she on any prescription medication or long-term steroid use. Her BMI was 20+, and she had been leading a healthy lifestyle. She was a non-smoker who drank alcohol socially and exercised 2 to 3 times a week. 

Clinical examination highlighted a circular midline smooth mound of soft tissue, which limited neck extension and mobility. It was situated below the nape of the neck and above the middle third of the back, extending into the interscapular region. 

Due to its mass effect, the full extension of the neck was limited with affected rotation. Cervicodorsal fat pad removal surgery via liposuction was proposed to the patient. The pros and cons of the surgery were explained to the patient in detail. A pre-surgery blood work was ordered. 

Treatment Plan

Informed consent was taken. The surgery was conducted at an accredited day surgery centre, where the patient was under twilight sedation and in a prone position. An anesthesiologist was present to monitor the patient's vital stats. At the same time, Dr Ivan Puah carried out the removal of the cervicodorsal fat pad with the assistance of surgically trained OT personnel. The anaesthetic solution was infiltrated into the targeted area via small incisions of less than 4mm. Following that, ultrasonic-assisted suction lipectomy was done to remove excess fat. Wounds were subsequently closed with stitches. 

Post-Op Recovery & Results 

Ms NX's post-operative recovery was smooth, with diligent scar management and adherence to post-operative care instructions. As the healing process unfolded, Ms NX experienced relief from discomfort and observed improvements in neck mobility. Follow-up appointments demonstrated positive outcomes, reaffirming the efficacy of the intervention in addressing Ms N's concerns and enhancing her quality of life.
