Patient's Profile

Mr M. M. is a Singaporean man in his early 30s who works in the public sector. He has been dealing with the uncomfortable and embarrassing condition of gynecomastia, the enlargement of male breasts, since his teenage years. He had tried many methods to eliminate this issue but to no avail. 

Background

Despite losing a significant amount of weight through dieting and exercise, the enlarged male breasts remained, and the excessive weight loss only made the condition more prominent, leaving him with loose skin and stretch marks around his chest area.

Encouraged by a friend, Mr M. M. first consulted Dr Ivan Puah for possible gynecomastia treatment in 2019. He was diagnosed with Grade III gynecomastia, with noticeable breast swelling and skin laxity.

At that time, Dr Puah recommended gynecomastia surgery to address the issue, but Mr M. M. opted to try managing the condition naturally with weight training.

By 2024, Mr M. M. found that despite his best efforts, including a rigorous weightlifting routine to bulk up and reduce the appearance of his chest, the gynecomastia had not improved or resolved.

He consulted Dr Ivan Puah again. During the clinical assessment, Mr M. M was alarmed to learn that his condition had progressed to Grade IV gynecomastia, with a significant amount of loose skin and stretch marks. Dr Puah recommended the patient undergo gynecomastia surgery, which he agreed to as  he had exhausted all other options.

Side effects, risks of surgical complications such as haematoma, scarring, and expected results were clearly explained to the patient. Before the surgery, the patient underwent a full pre-operative assessment, including blood tests and counselling. 

Treatment Plan

Mr M. M. underwent gynecomastia surgery at an accredited day surgery centre in 2024. 

The surgery proceeded smoothly. The patient was under twilight sedation, and vital stats were carefully monitored by the team's anaesthetist.  

During the surgery, Dr. Puah utilised his proprietary 360° GTD® (Gynecomastia Tissue Dissection) method to remove the glandular breast tissue associated with gynecomastia while ensuring minimal trauma to the surrounding area. This method allows for comprehensive and effective removal of the excess and enlarged breast tissue, which is critical in treating severe gynecomastia cases like Mr M. M.'s.

Dr Puah also employed the MDC-Sculpt® Lipo technique to sculpt the chest area three-dimensionally, removing residual fat tissues and tightening the skin for a more masculine contour. Combining these two methods ensures a more aesthetically pleasing outcome with minimal scarring and optimal results. 

The glandular tissue removed during the surgery was sent for biopsy to rule out any possible underlying conditions.  

Post-Surgery Recovery and Results

Mr M. M. began his recovery as planned. There was minimal bruising, pain, and discomfort, which was managed with the pain relief medications ordered by Dr Puah. 

He was advised about post-surgical care, including staying out of the sun, applying scar gel to the treated area, and wearing the surgical compression garment 24/7 for 8 weeks. Tissue swelling gradually improved over time, and the results were evident when Mr M.M.M returned to the clinic for a post-op review.

His chest is now more contoured, and he diligently applied scar gel and sunblock to his wounds.
