Mr S, a man in his mid-30s working in administration, has been experiencing bilateral breast enlargement, or gynecomastia, since his early teens.
Background
Despite losing a significant amount of weight during his National Service (NS), he continued to experience a condition that left him highly self-conscious about his appearance. Mr S rated the emotional and physical impact of his male chest enlargement as a 7 out of 10, which affected not only his confidence but also his daily activities.
Over the years, he tried to manage the issue through weight loss and exercise, but despite his efforts, the enlarged chest remained. Mr S felt stuck; no matter what he did, the condition wouldn’t go away. It wasn’t just the physical appearance that troubled him; the emotional toll it took on his self-esteem was particularly challenging, especially in social situations.
In early 2024, after receiving a recommendation from a friend who had successfully undergone gynecomastia surgery with Dr Ivan Puah, Mr S decided to come in for an evaluation.
Consultation & Assessment
During the consultation, Dr Puah diagnosed Mr S with Grade III gynecomastia, noting that he exhibited moderate skin laxity (Type 4/5). The excess breast tissue and fat had blurred the natural border of the pectoral muscles, giving his chest a rounded and somewhat feminine appearance.
Mr S's nipple had descended below the pectoralis border due to the skin laxity, resulting in a horizontal fold across his chest. The objective was to achieve a well-proportioned, V-shaped chest that Mr S could confidently display in fitted shirts. After careful consideration, he decided to proceed with the gynecomastia surgery. Dr Puah clearly explained the potential side effects, complications, and expected results to the patient. Blood tests were then conducted to ensure that he was fit for surgery.

Post-Op Recovery & Results
Mr S experienced minimal bruising and soreness after the surgery. His stitches were removed after 10 days, and Dr Puah advised him to wear a compression vest continuously for the next 8 weeks to aid in skin tightening and recovery.
The results were noticeable immediately following the surgery, and as the swelling gradually decreased over the next three months, Mr S's chest continued to improve. He also made a conscious effort to apply scar gel and sunblock on his scars whenever he was exposed to the sun. Over time, his incision scars healed well and became less noticeable.
