Back rolls and bra bulge liposuction are an art. It is more than a fat removal surgery to achieve smooth back contour results for high patient satisfaction.
Table of Contents
Back rolls and bra bulges refer to areas of excess fat that can develop around the upper back and the sides of the chest, particularly in women.
These fat deposits often become more noticeable when wearing snug-fitting clothing or bras. Back rolls typically appear as folds of skin or fatty tissue that may create a visible line along the back, while bra bulges occur when breast tissue and fat push themselves over the edges of the bra, resulting in an uneven bulging appearance.
The unattractive bulge may make a woman highly self-conscious about themselves and may lead to a negative body image.
Is back fat caused by bad posture? Or a bra?
Along with poorly fitted bras, back fat is also often linked to poor posture, as both can cause the skin to wrinkle and the affected area to bulge more prominently.
However, these factors are usually temporary causes.
Why is back fat so hard to lose, despite of exercise and calorie-deficit diet
The leading causes of back rolls and bra bulges include:
- the natural ageing process,
- a genetic predisposition to store fat in the back,
- excess fat pockets in the area,
- redundant excess skin [1], or
- atrophy of the back muscles.
Fat on upper back linked with diseases and increased risk of insulin resistance
Research showed that fat pockets on the chest and back, also referred to as upper trunk fat, are linked to a higher risk of insulin resistance, a condition that can lead to type II diabetes [2].
“We knew about the insulin resistance risk associated with visceral fat, which has been shown in previous studies, but no one had ever looked at the contribution of upper trunk fat,” says lead author and FRAM (Final Progress Report Additional Materials) principal investigator Carl Grunfeld, MD, PhD, chief of the metabolism and endocrine sections at San Francisco VA Medical Center.
The researchers discovered that both visceral and subcutaneous fat independently contributes to insulin resistance, regardless of the presence of the other type.
Scientifically proven successful back fat and bra bulge removal and contouring with Vaser Lipo (UAL) & Helium-activated radiofrequency
According to many previous studies on back liposuction, there are two primary reasons why they have been unsuccessful.
1. The back fat in this area is primarily superficial and not suitable for optimal removal using traditional liposuction techniques due to the limitation of the cannula’s precision in traditional liposuction [3].
2. Back rolls comprise skin redundancy [4], and traditional lipo would need to be combined with skin excision surgery to achieve a more successful back roll elimination that would cause prominent post-op scarrings [5, 6].
Mowlavi et al. studied 14 back contouring cases treated with simultaneous ultrasound-assisted liposuction (UAL) and helium-activated radiofrequency instead of excisional surgery to tighten the skin using subdermal coagulation. All 14 patients showed visible positive results – back rolls were removed, and the upper and middle back contour drastically improved – and “overall satisfaction in their postoperative follow-ups at 3, 6, and 12 months.” [7].
This signifies the successful combination of simultaneous ultrasound-assisted liposuction and helium-activated radiofrequency as an effective treatment for patients who want to remove back fat and improve back contour without excision surgery.

How back lipo is an art. Dr Ivan Puah is not just an MOH-accredited lipo doctor but also a body sculptor
"Having performed liposuction for over two decades, I concluded that liposuction is not just a surgical procedure to remove unwanted and excess fat. In my opinion, lipo doctors are like sculptors, and liposuction is a form of art. A patient’s result is influenced by not just the unique anatomy but also the liposuction doctor's ability to work and shape the body. It is important that the patient selects a lipo doctor based on the doctor's artistic flair, experiences and consistency in producing smooth back contour results for high patient satisfaction," shares Dr Ivan Puah, Chairman of Lipo Peer Review Committee Singapore.
Dr Puah, an accredited MOH liposuction doctor in Singapore, is an advocate that liposuction is 30% science and 70% art. He understands the patient's unique fat distribution and meticulously sculpt the targeted areas with precise navigation through the different fat layers by using the suitable liposuction techniques tailored to the patient’s body shape. The objective is to remove excess and unwanted fat tissues, stimulate collagen reproduction, tighten skin and enhance the patient’s natural silhouette while keeping surgical scarring short and minimal.
